Physicians verification is required for those applying with a disability.
Schedule trips one (1) day in advance from 8:30am-noon. Requests for transportation on Saturday, Sunday, and Monday are accepted on the preceding Friday.
Transportation is provided on first-come, first-served basis.
Monday and Tuesdays in limited areas of Glendale and North Hollywood for medical appointments only.
